Background:
Social connection is associated with better health, including reduced risk of dementia. Personality traits are also linked to cognitive outcomes; neuroticism is associated with increased risk of dementia. Personality traits and social connection are also associated with each other. Taken together, evidence suggests the potential impacts of neuroticism and social connection on cognitive outcomes may be linked. However, very few studies have simultaneously examined the relationships between personality, social connection and health.
Research objective:We tested the association between neuroticism and cognitive measures while exploring the potential mediating roles of aspects of social connection (loneliness and social isolation).
Method:We conducted a cross-sectional study with a secondary analysis of the Canadian Longitudinal Study on Aging (CLSA) Comprehensive Cohort, a sample of Canadians aged 45 to 85 years at baseline. We used only self-reported data collected at the first follow-up, between 2015 and 2018 (n= 27,765). We used structural equation modelling to assess the association between neuroticism (exposure) and six cognitive measures (Rey Auditory Verbal Learning Test immediate recall and delayed recall, Animal Fluency Test, Mental Alternation Test, Controlled Oral Word Association Test and Stroop Test interference ratio), with direct and indirect effects (through social isolation and loneliness). We included age, education and hearing in the models and stratified all analyses by sex, females (n= 14,133) and males (n=13,632).
Preliminary results of the ongoing study:We found positive, statistically significant associations between neuroticism and social isolation (p<0.05) and loneliness (p<0.05), for both males and females. We also found inverse, statistically significant associations between neuroticism and all cognitive measures (p<0.05), except the Stroop Test interference ratio. In these models, there was consistent evidence of indirect effects (through social isolation and loneliness) and, in some cases, evidence of direct effects. We found sex differences in the model results.
Conclusion:Our findings suggest that the association between neuroticism and cognitive outcomes may be mediated by aspects of social connection and differ by sex. Understanding if and how modifiable risk factors mediate the association between personality and cognitive outcomes would help develop and target intervention strategies that improve social connection and brain health.
A peer support dietary change intervention for encouraging adoption and maintenance of the Mediterranean diet in a non-Mediterranean population (TEAM-MED): lessons learned and suggested improvements

Peer support interventions for dietary change may offer cost-effective alternatives to interventions led by health professionals. This process evaluation of a trial to encourage the adoption and maintenance of a Mediterranean diet in a Northern European population at high CVD risk (TEAM-MED) aimed to investigate the feasibility of implementing a group-based peer support intervention for dietary change, positive elements of the intervention and aspects that could be improved. Data on training and support for the peer supporters; intervention fidelity and acceptability; acceptability of data collection processes for the trial and reasons for withdrawal from the trial were considered. Data were collected from observations, questionnaires and interviews, with both peer supporters and trial participants. Peer supporters were recruited and trained to result in successful implementation of the intervention; all intended sessions were run, with the majority of elements included. Peer supporters were complimentary of the training, and positive comments from participants centred around the peer supporters, the intervention materials and the supportive nature of the group sessions. Attendance at the group sessions, however, waned over the intervention, with suggested effects on intervention engagement, enthusiasm and group cohesion. Reduced attendance was reportedly a result of meeting (in)frequency and organisational concerns, but increased social activities and group-based activities may also increase engagement, group cohesion and attendance. The peer support intervention was successfully implemented and tested, but improvements can be suggested and may enhance the successful nature of these types of interventions. Some consideration of personal preferences may also improve outcomes.

Summary
‘E-learning’ can be defined broadly as the use of internet technologies to deliver teaching and to enhance knowledge and performance. It is also referred to as web-based, online, distributed or internet-based learning (Ruiz et al. 2006). Many sites use ‘blended learning’, where e-learning is combined with in-person or virtual face-to-face instructor-led training.
The increase in portability, power and connectivity of devices means that most smartphones can easily access information in real time (Marzano et al. 2017) and, of internet users worldwide, 93% access the internet via mobile devices (Johnson 2021). This means that access to the internet to gather information about mental health is immediate, but the vast number of information sites can easily become overwhelming for both patients and clinicians. A simple search for a single mental health topic generates a huge number and range of results. These vary from reviews of the evidence and primary research articles, to news articles and advertisements for treatment centres. The internet user is swamped with an array of sites of variable (and often unknown) quality, which are neither necessarily relevant to the original question nor ranked in order of reliability.

As the COVID-19 pandemic took hold in the USA in early 2020, it became clear that knowledge of the prevalence of antibodies to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) among asymptomatic individuals could inform public health policy decisions and provide insight into the impact of the infection on vulnerable populations. Two Clinical and Translational Science Award (CTSA) Hubs and the National Institutes of Health (NIH) set forth to conduct a national seroprevalence survey to assess the infection’s rate of spread. This partnership was able to quickly design and launch the project by leveraging established research capacities, prior experiences in large-scale, multisite studies and a highly skilled workforce of CTSA hubs and unique experimental capabilities at the NIH to conduct a diverse prospective, longitudinal observational cohort of 11,382 participants who provided biospecimens and participant-reported health and behavior data. The study was completed in 16 months and benefitted from transdisciplinary teamwork, information technology innovations, multimodal communication strategies, and scientific partnership for rigor in design and analytic methods. The lessons learned by the rapid implementation and dissemination of this national study is valuable in guiding future multisite projects as well as preparation for other public health emergencies and pandemics.
Drivers of farmer involvement in experimental forage trials in the Peruvian Andes and implications for participatory research design

This study analyses the experience and response of farmers within a multi-year collaborative research trial focused on the development of forage-based fallows in eight communities in the central Peruvian Andes. Quantitative data from a rural household survey were used to characterize farming household socioeconomic factors, livelihood strategies and soil and crop management practices of community members belonging to four participation groups with respect to the trials: 1) current participants near the end of the trial; 2.) those who participated early on, but dropped the trials after the first year; 3) those who participated in meetings but not directly in experiments; and 4) those who never participated meaningfully in the process. Furthermore, qualitative interviews of farmers in the four groups were used to examine trends and questions arising from the quantitative survey findings. Analysis of this mixed-methods dataset showed that better resource-endowed households (in terms of human and social capital, more livestock assets, higher levels of farm value production and income, and farm inputs) tended to be more likely to participate compared to households with lower levels of these variables. Our findings suggest that the differences in resource endowment among participation group households may be related to household life cycles, where access to resources change over time, reflecting the changing demography of a household. It was established that farm households with intermediate-age children, that is near the middle of a farm life cycle trajectory, are those with the most wherewithal to participate in trials and likely serve as examples and test cases for other farms with younger parents or older farmers with children moved away. Follow-up interviews indicated that farming households at either end of the farm life cycle trajectory may be using a ‘wait-and-see’ approach to the trials carried out by their neighbours who have more labour and other resources to deploy. In light of these findings, we suggest that participatory research should aim to ensure that the voices, challenges and opportunities of Non-participants are represented in the research process and experimental design. Additionally, greater consideration should be placed on understanding management by context issues in order to better target potential farming innovations such as improved fallows, at multiple levels, from the field to the household and to the community and beyond.

The Hierarchical Taxonomy of Psychopathology (HiTOP) has emerged out of the quantitative approach to psychiatric nosology. This approach identifies psychopathology constructs based on patterns of co-variation among signs and symptoms. The initial HiTOP model, which was published in 2017, is based on a large literature that spans decades of research. HiTOP is a living model that undergoes revision as new data become available. Here we discuss advantages and practical considerations of using this system in psychiatric practice and research. We especially highlight limitations of HiTOP and ongoing efforts to address them. We describe differences and similarities between HiTOP and existing diagnostic systems. Next, we review the types of evidence that informed development of HiTOP, including populations in which it has been studied and data on its validity. The paper also describes how HiTOP can facilitate research on genetic and environmental causes of psychopathology as well as the search for neurobiologic mechanisms and novel treatments. Furthermore, we consider implications for public health programs and prevention of mental disorders. We also review data on clinical utility and illustrate clinical application of HiTOP. Importantly, the model is based on measures and practices that are already used widely in clinical settings. HiTOP offers a way to organize and formalize these techniques. This model already can contribute to progress in psychiatry and complement traditional nosologies. Moreover, HiTOP seeks to facilitate research on linkages between phenotypes and biological processes, which may enable construction of a system that encompasses both biomarkers and precise clinical description.
The contribution of depressive ‘disorder characteristics’ to determinations of prognosis for adults with depression: an individual patient data meta-analysis

Background
This study aimed to investigate general factors associated with prognosis regardless of the type of treatment received, for adults with depression in primary care.
MethodsWe searched Medline, Embase, PsycINFO and Cochrane Central (inception to 12/01/2020) for RCTs that included the most commonly used comprehensive measure of depressive and anxiety disorder symptoms and diagnoses, in primary care depression RCTs (the Revised Clinical Interview Schedule: CIS-R). Two-stage random-effects meta-analyses were conducted.
ResultsTwelve (n = 6024) of thirteen eligible studies (n = 6175) provided individual patient data. There was a 31% (95%CI: 25 to 37) difference in depressive symptoms at 3–4 months per standard deviation increase in baseline depressive symptoms. Four additional factors: the duration of anxiety; duration of depression; comorbid panic disorder; and a history of antidepressant treatment were also independently associated with poorer prognosis. There was evidence that the difference in prognosis when these factors were combined could be of clinical importance. Adding these variables improved the amount of variance explained in 3–4 month depressive symptoms from 16% using depressive symptom severity alone to 27%. Risk of bias (assessed with QUIPS) was low in all studies and quality (assessed with GRADE) was high. Sensitivity analyses did not alter our conclusions.
ConclusionsWhen adults seek treatment for depression clinicians should routinely assess for the duration of anxiety, duration of depression, comorbid panic disorder, and a history of antidepressant treatment alongside depressive symptom severity. This could provide clinicians and patients with useful and desired information to elucidate prognosis and aid the clinical management of depression.
Stop the Bleed: An Interprofessional Community Service Learning Project Assessing the Efficacy of Pharmacist-Led Hemorrhage Control Education for Laypersons

Objectives:
Stop the Bleed (STB) is a national initiative that provides lifesaving hemorrhagic control education. In 2019, pharmacists were added as health-care personnel eligible to become STB instructors. This study was conducted to evaluate the efficacy of pharmacist-led STB trainings for school employees in South Texas.
Methods:Pharmacist-led STB trainings were provided to teachers and staff in Laredo, Texas. The 60-min trainings included a presentation followed by hands-on practice of tourniquet application, wound-packing, and direct pressure application. Training efficacy was assessed through anonymous pre- and postevent surveys, which evaluated changes in knowledge, comfort level, and willingness to assist in hemorrhage control interventions. Student volunteers (predominantly pharmacy and medical students) assisted in leading the hands-on portion, providing a unique interprofessional learning opportunity.
Results:Participants with previous training (N = 98) were excluded, resulting in a final cohort of 437 (response rate 87.4%). Compared with baseline, comfort level using tourniquets (mean, 3.17/5 vs 4.20/5; P < 0.0001), opinion regarding tourniquet safety (2.59/3 vs 2.94/3; P < 0.0001), and knowledge regarding tourniquets (70.86/100 vs 75.84/100; P < 0.0001) and proper tourniquet placement (2.40/4 vs 3.15/4; P < 0.0001) significantly improved.
Conclusions:Pharmacist-led STB trainings are efficacious in increasing school worker knowledge and willingness to respond in an emergency hemorrhagic situation.
Potential feedbacks between loss of biosphere integrity and climate change

Individual organisms on land and in the ocean sequester massive amounts of the carbon emitted into the atmosphere by humans. Yet the role of ecosystems as a whole in modulating this uptake of carbon is less clear. Here, we study several different mechanisms by which climate change and ecosystems could interact. We show that climate change could cause changes in ecosystems that reduce their capacity to take up carbon, further accelerating climate change. More research on – and better governance of – interactions between climate change and ecosystems is urgently required.

Collaborative quality improvement and learning networks have amended healthcare quality and value across specialities. Motivated by these successes, the Pediatric Acute Care Cardiology Collaborative (PAC3) was founded in late 2014 with an emphasis on improving outcomes of paediatric cardiology patients within cardiac acute care units; acute care encompasses all hospital-based inpatient non-intensive care. PAC3 aims to deliver higher quality and greater value care by facilitating the sharing of ideas and building alignment among its member institutions. These aims are intentionally aligned with the work of other national clinical collaborations, registries, and parent advocacy organisations. The mission and early work of PAC3 is exemplified by the formal partnership with the Pediatric Cardiac Critical Care Consortium (PC4), as well as the creation of a clinical registry, which links with the PC4 registry to track practices and outcomes across the entire inpatient encounter from admission to discharge. Capturing the full inpatient experience allows detection of outcome differences related to variation in care delivered outside the cardiac ICU and development of benchmarks for cardiac acute care. We aspire to improve patient outcomes such as morbidity, hospital length of stay, and re-admission rates, while working to advance patient and family satisfaction. We will use quality improvement methodologies consistent with the Model for Improvement to achieve these aims. Membership currently includes 36 centres across North America, out of which 26 are also members of PC4. In this report, we describe the development of PAC3, including the philosophical, organisational, and infrastructural elements that will enable a paediatric acute care cardiology learning network.
Supportive release techniques provide no reintroduction benefit when efficacy and uptake is low

Release methods can influence the outcome of reintroductions. We tested the effect of delayed, immediate and supplementary food/shelter release treatments on the reintroduction of brushtail possums Trichosurus vulpecula to an environment in which introduced predators, particularly foxes, were subject to control. Monitoring of 48 radio-collared possums over 3 months revealed that immediate release possums settled into a stable range significantly faster than other groups, but there were no differences in survival, dispersal distance, reproduction or body condition. Ten days after release possums from all treatment groups had lost body mass, but by day 60 most were heavier than at the time of translocation. After release, possums sometimes used shelter sites easily accessible to predators, but within 3 weeks they regularly selected safer shelter. Risky shelter selection and loss of condition immediately after release suggests that supplementary food and shelter could be beneficial, but supportive measures were rarely used or did not have the desired effect. In an environment with higher predator densities, risky shelter selection could lead to high post-release predation, and mass loss could encourage animals to forage in riskier ways, further increasing vulnerability. In these environments effective uptake of supplementary food and shelter could reduce predation risk, but supplementary measures would need to be presented in a way that maximises uptake. In contrast, if post-release predation risk is low then supportive measures may not be required. Innovative methods for providing post-release support should continue to be developed for reintroductions to areas where supportive measures are needed.

The common edible fig is a subcanopy tree that has invaded many of the remnant riparian forests of California's Central Valley. Fig is unusual in its ability to invade low-light, low-disturbance, native-plant–dominated environments. Dendrochronology combined with regression and spatial analyses allowed us to empirically quantify the expansion rate and spatial pattern of the fig invasion into the native plant community at Caswell Memorial State Park (Ripon, CA) over a 70-year invasion period. Fig uses a combination of short-distance dispersal, which results in constant, linear expansion at source population sites and long-distance dispersal, which eventually leads to high recruitment of satellite populations in ideal environments. Although fig initially experienced a long lag in its invasion rate, at the time of this study, it was expanding at an exponential rate at the landscape scale in Caswell. We identified a number of characteristics intrinsic to the fig population (shade suppression, pollinator presence, highly specialized reproduction, and propagule pressure) as well as extrinsic characteristics of the receiving environment (hydrologic alteration from the construction of a dam, safe sites for juvenile recruitment, and target effects from environmental heterogeneity) that may have influenced the rate and pattern of fig invasion. The Central Valley riparian forests have been reduced to less than 6% of their original area, and invasive fig is a significant threat to the remaining fragments of this important vegetation community. We include suggestions for fig eradication based on knowledge gained in this study.

In situ small-angle x-ray scattering (SAXS) is used to investigate the electrochemical durability of Pt-Metal (Pt-M) catalysts sputtered onto nitrogen-modified high surface area carbon powder. The results demonstrate that nitrogen modification promotes catalyst durability through reduction of nanoparticle dissolution and coarsening. Although particle sizes of Pt-M on high surface area carbon supports can be difficult to determine with transmission electron microscopy (TEM), a novel SAXS method has been employed to calculate particle size. SAXS analysis shows that the Pt-M nanoparticle size distribution remained stable for 3000 electrochemical cycles after nitrogen modification, whereas the unmodified support material leads to Pt-M nanoparticle instabilities. These results for industrial-relevant catalyst/support architectures underscore the potential of nitrogen-modified carbon support structures for enhanced Pt-M catalyst durability.
